Omni Family of Services Kentucky Inc seeks a Community Clinical Coordinator to provide direct services to clients and their families within the FITT program. You will support the FITT Director and coordinate care for youth at risk of out-of-home placement in a 90-day intensive program.

The role emphasizes trauma-responsive, evidence-based care, collaborative teamwork, and a commitment to safety, healing, and growth for families and communities.

Position: Community Clinical Coordinator

As the Community Clinical Coordinator you would be responsible providing direct services to clients and their families as well as serving as a significant support to the FITT Director. The Family Intervention Treatment Team (FITT) is an intensive, 90-day program, for youth who are at risk of out-of-home placement (e.g., hospitalization, foster care disruptions/support, residential, detention).
