Delco Pa is seeking a County Caseworker 2 to locate, coordinate, and monitor essential services that support participants in daily life. You will help individuals access community resources and ensure services align with their Individual Support Plan.

Responsibilities include assisting participants in finding and arranging services, coordinating ISP development with participants, families, and providers, and maintaining regular contact to verify delivery of supports.

Qualifications

• Bachelor's degree with social welfare major or relevant coursework in social sciences.
• Equivalent combination of experience and training with 12 credits in sociology, social welfare, psychology, gerontology, criminal justice, or related social sciences.

Responsabilités

• Locate, coordinate, and monitor services identified in the Individual Support Plan.
• Assist participants and families in finding, arranging, and obtaining services.
• Coordinate development and maintenance of the Individual Support Plan with participants, families, team members, and providers.
• Maintain regular contact to ensure supports are delivered as outlined in the plan.
• Provide guidance to help participants access community resources and direct services if chosen.
