Job summary

The Town of Ashland is seeking a Case Manager to support residents in navigating social, emotional, material, financial, and environmental stressors through case management, referrals, and social support.

Under the Department of Prevention & Human Services, you will conduct intakes, coordinate with partners, and document encounters in the MySeniorCenter system, while fostering accessible, compassionate service.

Qualifications

  • Bachelor's Degree in Social Work, Psychology, or Human Services related field
  • Master's Degree in Social Work preferred
  • 1-3 years of progressively responsible experience in a human service related agency
  • Familiarity with community resources and services in the area
  • Ability to express compassion, maintain confidentiality, and assess/intake skills
  • Strong interpersonal skills, written and oral communication
  • Bilingual candidates proficient in Spanish or Portuguese encouraged

Responsibilities

  • Screen and refer residents for resources and supports
  • Assist residents with navigating SNAP/EBT, Fuel Assistance, and emergency funds
  • Conduct intakes and needs assessments, identify resources and next steps
  • Build professional relationships and provide information, referrals, and connections
  • Maintain documentation in the MySeniorCenter database and ensure timely notes
  • Engage with community partners and services to strengthen connections for residents
  • Support department social media presence and participate in relevant initiatives
  • Collaborate with Town departments and providers to meet resident needs
  • Promote LGBTQ+ affirming, anti-racist, inclusive service delivery
