Job summary

The Town of Ashland seeks a Case Manager to assist residents in accessing resources and navigating social services within the Department of Prevention & Human Services. The role emphasizes intake, referrals, and connecting clients to community supports.

A successful candidate will have a Bachelor's in Social Work, Psychology, or related field, with 1–3 years of experience. Strong interpersonal skills, confidentiality, and the ability to document in the MySeniorCenter system are essential.

Qualifications

  • Bachelor's degree in Social Work, Psychology or Human Services related field; Master’s in Social Work preferred.
  • 1-3 years of progressively responsible experience in a human service related agency.
  • Ability to understand mobility, poverty and related factors affecting residents.

Responsibilities

  • Serve as primary applicant screener and liaison to the Ashland Emergency Fund.
  • Liaise with Ashland Food Pantry to ensure continuum of patron services.
  • Assist residents with SNAP/EBT benefit applications.
  • Assist residents with Fuel Assistance applications.
  • Support Holiday Assistance Program and Thanksgiving Meal Program implementations.
  • Participate in Hoarding Task Force and Substance Response and Support Team activities.
  • Conduct intakes and needs assessments; provide referrals and follow-up.
