Job summary

The Elizabeth Layton Center is seeking 2 qualified Children's Case Managers to join our CBS team in Paola, KS. The role focuses on delivering case management and therapeutic services to youth with severe emotional disturbance, in family and community settings with direct supervision from the CBS Coordinator.

Responsibilities include treatment planning, resource coordination, and education for families, while maintaining formal documentation and collaborating with other center staff.

Qualifications

• Bachelor’s degree or equivalent experience meeting KS licensing requirements.
• Familiarity with mental health theory and practice.
• Clean driving record and ability to obtain Medicaid required security checks.

Responsibilities

• Participates in treatment planning based on best practice theories and applies interventions.
• Develops personal plan/goal sheets with consumer needs.
• Obtains resources and coordinates with area agencies.
• Advocates in Education/Vocation, Residential, Financial, Health, Leisure and Social Supports.
• Provides family education on symptoms, relapse signs and crisis handling.
• Delivers timely program documentation and reports to supervisor.
• Ensures transportation to program activities and maintains driving record.
• Meets direct service time and completes daily paperwork and deadlines.
• Provides supportive intervention beyond scheduled hours as needed.
