Job summary

Elizabeth Layton Center, Inc. is seeking a Case Manager to join the Community Support Services (CSS) team in Paola, KS. The role focuses on delivering case management and therapeutic services to individuals with complex mental health needs in community settings.

You will work closely with therapeutic staff to support independence and community inclusion, including SOAR case management for Social Security Benefits. A valid driver’s license and Kansas licensing familiarity are preferred.

Qualifications

• Relates a positive approach to complex mental health issues.
• Ability to communicate effectively orally and in writing.
• Ability to establish effective working relationships with staff, community agencies, and consumers.
• Ability to be sensitive to cultural and ethnic differences.

Responsibilities

• Participates in treatment planning based on best practice theories and applies interventions.
• Development of personal plan/goal sheet in conjunction with consumer's needs.
• Obtains location and the process of acquiring resources; coordinates with area agencies.
• Direct involvement in planning and advocacy in areas of Education/Vocation, Residential, Financial, Health, Leisure, and Social Supports.
• Education to family in symptomatology, awareness of relapse indicators and handling of crisis.
• Assists in collection of program data and provides reports as necessary to immediate supervisor.
• Meets direct service expectations and deadlines for daily paperwork and reports.
• Provides quality documentation and SOAR case management for clients seeking Social Security Benefits.
