Qualifications

• Strong oral and written communication skills in English.
• Valid driver’s license and automobile liability insurance.
• Basic personal computer skills for data entry and retrieval.

Responsibilities

• Provide services in the community where the consumer needs them (school, work, home, shelter, store, etc).
• Provide outreach, education, support, referrals and connection to resources for the consumer and family.
• Engage consumers with assertive outreach to improve outcomes such as housing stability and reduced hospitalizations.
• Develop and document treatment goals with input from client, family, and treatment team; monitor progress.
• Meet administrative and clinical documentation requirements in a timely manner.
• Attend clinical supervision and daily program staffing; collaborate with the team on issues.
