Elizabeth Layton Center Inc in Paola, KS seeks a dedicated CSS Case Manager to provide case management and therapeutic services to consumers with severe mental illness. You will coordinate with clinicians, educate clients, and support independence and community inclusion.

The role focuses on home and community-based services with minimal center time, reporting to the CSS Director and driving client outcomes through collaboration and advocacy.

• Relates a positive approach to complex mental health issues.
• Ability to communicate effectively orally and in writing.
• Ability to establish effective working relationships with staff, community agencies, and consumers.
• Ability to be sensitive to cultural and ethnic differences.
• Ability to safely perform major job responsibilities, with or without reasonable accommodation.

Mansioni

• Participates in treatment planning based on best practice theories.
• Develops personal plan/goal sheet in conjunction with consumer's needs.
• Obtains location and process of acquiring resources; coordinates with area agencies.
• Direct involvement in planning and advocacy in Education/Vocation, Residential, Financial, Health, Leisure, and Social Supports.
• Meets deadlines for daily paperwork and reports.
• Provides SOAR case management for clients seeking Social Security Benefits.
