Job summary

Washington County Mental Health Services (WCMHS) is seeking an Integrated Health Case Manager to provide community-based case management and support services to adults receiving mental health services in Vermont. You will collaborate with clients, families, clinicians, and community partners to help individuals access essential resources and work toward treatment goals.

You will conduct outreach in community settings, assist with benefits access, housing and transportation, and maintain

Qualifications

• Bachelor’s degree in social work, human services, or a related field is required.
• One year of service delivery experience with adults is required.
• Trauma-informed approach is valued.
• Valid driver’s license and reliable vehicle required.
• Ability to deliver services and complete treatment notes in English.

Responsibilities

• Help clients access medical, social, educational, vocational, housing, financial, and other essential services.
• Collaborate with clients to implement treatment goals and develop independent living skills.
• Develop natural support networks aligned with treatment goals.
• Coordinate with case managers, clinicians, community partners, and other team members; participate in meetings and supervision.
• Provide community-based support, including outreach visits and transportation when clinically indicated.
