Job summary

Institute for Community Living Inc. in New York, NY seeks a field-based Mental Health/Direct Services Professional to provide home visits, counseling, and service coordination for individuals with mental health needs.

You will conduct assessments, develop individualized plans, and coordinate with care managers and external providers to help consumers access medical and social resources in community settings.

Qualifications

• Bachelor’s degree in a human services field is preferred.
• Associate’s degree in a human services field with two years of direct service experience acceptable.
• High school diploma/GED with four years of direct human services experience accepted.

Responsibilities

• Conduct home visits to provide supportive counseling and independent living assistance.
• Help consumers modify behaviors, build insight, and develop coping skills.
• Assess needs and develop individualized service plans with documentation.
• Maintain case records, assessments, and quarterly reviews.
• Provide 1:1 and group counseling using cognitive-behavioral techniques.
• Coordinate services with care managers, ICL staff, and providers.
• Advocate for consumer needs and access to resources.
• Perform crisis assessment and intervention; accompany to appointments.
• Report incidents and conduct safety checks of residences.
• Plan and facilitate recreational or social activities.
