Job summary

The role is primarily community-based and involves guiding clients toward greater independence. Responsibilities include assessments, recovery planning, and collaboration with clinical and social service partners.

Qualifications

• Bachelor’s degree and 1–2 years of mental health/substance abuse experience.
• Masters degree may be considered in lieu of general experience.
• Bi-lingual Spanish preferred.
• Experience doing case notes and IRPs using Carelogic are advantageous.
• Valid driver’s license, reliable vehicle, current registration and insurance.

Responsibilities

• Provides a full range of community support services including case management and housing.
• Completes timely rehabilitation assessments and updates recovery plans.
• Engages clients to increase involvement in learning, employment and social activities.
• Develops relationships with clinical, educational, vocational and social service agencies.
• Documents required paperwork and provides transportation for clients as needed.
• Facilitates self‑help and advocacy groups and participates in required meetings.
• Adheres to Chrysalis Center, Inc.’s policies and performs other duties as assigned.
