Qualifications

Responsibilities

  • Provides transportation to clients as appropriate using agency vehicle.
  • Attends and participates in all required meetings.
  • Adheres to Chrysalis Center, Inc.’s policies and maintains professional standards.
  • Performs other duties as assigned.

POSITION SCOPE:

This position will provide community support and individual interventions based on assessment and evaluation; provide linkage and referral based on individual need and as required by funded agency programs in the Recovery and Empowerment Center. Responsibilities will primarily consist of the provision of direct care in a group setting; generally consist of guiding, assisting and supporting individuals who have complex psychiatric, substance abuse, medical, social and legal issues to improve daily living skills, improve level of functioning, enhance independence and promote recovery and community integration. This position may include development and implementation of a full range of therapeutic, recreational, vocational and social activities. Hours may vary and may include evening, holidays, and weekends.

ESSENTIAL RESPONSIBILITIES
  1. 1. Provides a full range of community and social support services to an assigned caseload of individuals.
  2. 2. Completes timely rehabilitation assessment, reassessments and completes/updates recovery plans.
  3. 3. Engages clients and works in partnership to increase or maintain involvement in learning, working and social activities.
  4. 4. Pursues best practice models for clients in continuing education, employment and socialization.

  10. 10. Performs other duties as assigned.
Qualifications
  1. 1. Bachelor’s degree required plus a minimum of one to two years of experience in mental health and substance abuse fields.
  2. 2. Bi-lingual Spanish, preferred.
  3. 3. Experience in working with diverse populations in a group setting.
  4. 4. Proficiency managing Microsoft and other computer applications used for tracking data and reporting outcomes.
  5. 5. Easily adapts to change and competing priorities.
  6. 6. Ability to de-escalate crisis situation with professionalism and sound judgment.
  7. 7. Possess and maintains a valid Driver’s License, reliable vehicle, current registration and insurance.
  8. 8. Ability to effectively interact with clients and members of various teams within and outside the agency.

  9. 9. Effectively communicates and presents information clearly and in a concise manner in verbal and written format.
  10. 10. Demonstrates excellent interpersonal skills including professional demeanor and positive, service orientated attitude toward all internal and external customers.
  11. 11. Demonstrates ability to adhere to strict Confidentiality and HIPAA Guidelines.
  12. 12. Provides guidance, assistance and support to clients as well as works collaboratively with Chrysalis Team.
