Job summary

CrossWinds Counseling & Wellness in Emporia, KS is seeking a full-time Case Manager who provides quality case management services to clients, leveraging a strength-based approach to help them adapt to life situations and learn independent living skills.

The role focuses on coordinating resources, facilitating psychosocial groups where applicable, and working with clients to develop individualized plans of care while maintaining accurate records and meeting licensure requirements.

Qualifications

• Associate’s degree plus two years’ experience in the health and human services field OR four years’ experience in the health and human services field is required.
• Bachelor’s degree in psychology, social work, or counseling is preferred.

Responsibilities

• Provide direct case management services to children, usually on a weekly basis, to help integrate individuals into the community.
• Meets or exceeds the billable hour standard of client contact hours set for position.
• Assist clients to identify, secure, and maintain community resources.
• Coordinate referrals and delivery of services among community agencies, employment contacts, and the organization to ensure needed services.
• Work with clients and others to design and implement a comprehensive plan of care based on strengths.
• Maintain accurate records of all client services and submit reports timely.
• Develop knowledge of community resources and how to access these services for clients’ benefit.
• Establish and maintain positive relationships with clients and partners; attend workshops to maintain licensure.
