Job summary

Wyandot Center is seeking a Case Manager to provide goal-directed services to young adults (18–26) with serious mental illness. You will help increase independent functioning, support group attendance, and collaborate with the EPIC Team for first-episode psychosis care.

Based in Kansas City, the role requires community outreach, strong communication, and the ability to work with diverse populations. Regular travel in the region and thorough documentation are expected.

Qualifications

• Bachelor’s degree in social work, psychology, rehabilitation or related field.
• Preferred experience in mental health, community-based or outreach work.
• Ability to build relationships with clients and community partners.
• Proficient in Microsoft Office; reliable transportation and valid driver’s license.

Responsibilities

• Engage individuals and families as partners in services and develop positive, hopeful, and trusting relationships.
• Participate as an active member of the service provider team.
• Conduct assessments and participate in treatment plan or plan of care development.
• Mediate between clients/families and other professionals directing services and care.
• Maintain documentation and reporting requirements thoroughly and on time.
• Regularly travel and transport clients in and around the region, and provide direct care and support as needed.
