Job summary

The Lakes Region Mental Health Center, Inc. seeks a Clinical Professional to coordinate and assure continuity of services for clients on a caseload. This full-time role focuses on intake assessments, service planning, and linkage to supportive resources.

You will work with clients and caregivers, document in compliance with state policies, attend meetings, and uphold person-centered planning and quality standards in a day shift environment in New Hampshire.

Qualifications

• Bachelor's degree in a mental health field with two years of related experience.
• NH driver's license with current liability insurance required.
• Must demonstrate crisis intervention and counseling principles and maintain up-to-date practices.

Responsibilities

• Coordinate services and assure continuity of care for caseload clients.
• Meet with clients/caregivers at intake and during periodic reviews to assess needs.
• Document and bill in compliance with state and agency policy.
• Attend meetings, trainings, and supervision as required.
• Refer and monitor client services and communicate with providers.
