Aging Services of North Central Massachusetts (ASNCM) is seeking a Community Case Manager – Older Adult Services to join our State Home Care team.
This position is ideal for professionals who enjoy building meaningful relationships, working independently, connecting consumers with community resources, and helping older adults remain safely in their homes.
Rather than sitting behind a desk all day, you will work directly with consumers, family members, caregivers, healthcare providers, and community partners to assess needs, develop service plans, and coordinate services that improve quality of life and support independent living. The State Home Care Case Manager role includes assessing needs, developing service plans, coordinating resources, monitoring progress, and maintaining program documentation.
Build lasting relationships with older adults, caregivers, healthcare providers, and community partners while helping individuals remain independent and connected to their communities.
Every day you will help older adults access the services and supports they need to live safely and successfully at home.
Enjoy a combination of consumer visits, community engagement, care coordination, service planning, and problem-solving.
Balance community-based work, consumer visits, and administrative responsibilities.
ASNCM reimburses approved business travel at the current federal mileage reimbursement rate.
