Job summary

Minuteman Senior Services seeks a Care Manager to maintain a caseload of older adults and individuals with disabilities living in the community. You will visit homes, assess needs, determine eligibility, and coordinate service plans with a multidisciplinary team.

You will work with agency nurses and community facilities, ensure timely documentation, and cover shifts when a Care Manager is unavailable. Valid driver’s license and reliable car are required.

Qualifications

• Bachelor’s degree preferred; equivalent work experience may be considered.
• Experience using MS Office (Word, Outlook) required.
• Experience with older adult and disabled population preferred.
• Reliable car, valid driver's license, and car insurance required.

HOURS PER WEEK: 35GENERAL SUMMARY:The Care Manager is responsible for maintaining a caseload of older adult and individuals with disabilities who live in the community. Responsibilities include conducting home visits, assessing consumer needs, determining program eligibility, developing and coordinating service plans, and updating consumer records in a timely manner. Working as part of an interdisciplinary team, the Care Manager collaborates with agency nurses and community nursing facilities.ESSENTIAL FUNCTIONS:Conduct in-home functional assessments of consumers.Develop service plans with the consumer and their support network and monitor their ongoing effectiveness.Arrange services through contracted vendors and other community resources.Assess consumers for risk, abuse and neglect and report such issues as needed.Maintain case documentation in accordance with AGE documentation standards.Assume responsibility for covering Care Manager duties several times per month assisting consumers whose Care Manager is unavailable.Participate in Staff Development including staff meetings and trainings.COMPETENCIES NEEDED:Accountability: Demonstrates a high level of commitment to achieving results. Employees work in varied residential environments based on assigned caseload.
