Job summary

Brio Living Services in Monroe, MI is seeking a Community Care Registered Nurse (RN) to provide comprehensive nursing care to PACE participants across the continuum of care. The RN will act under supervision while applying advanced clinical judgment to support safe, person-centered outcomes.

Responsibilities include assessments, care planning, implementing treatments, coordinating with the IDT, and educating families about 24-hour care services.

Qualifications

• Graduate of an accredited School of Nursing.
• Current Registered Nurse (RN) license in Michigan.
• CPR certification required (or ability to obtain upon hire).
• Valid Michigan driver’s license with reliable transportation.
• Ability to lift and move up to 35 pounds independently.

Responsibilities

• Provide direct and indirect nursing care to PACE participants.
• Coordinate care and participate in IDT meetings.
• Perform nursing assessments and develop care plans.
• Administer treatments, IV therapy, wound care and medication management.
• Educate participants and families on 24-hour care services.
• Participate in after-hours on-call rotation and home visits.
