Job summary

Brio Living Services is seeking aCommunity Care RN in Ypsilanti, MI. This exempt full-time role provides both direct and indirect nursing care to PACE participants under supervision, coordinating across the care continuum.

The RN will perform assessments, develop care plans, administer treatments, manage medications, and participate in on-going quality improvement and after-hours on-call rotations. Michigan RN license and CPR required.

Qualifications

• Graduate of an accredited School of Nursing.
• Current Registered Nurse (RN) license in the State of Michigan.
• CPR certification required (or ability to obtain upon hire).
• Valid Michigan driver’s license with reliable transportation.
• Ability to lift and move up to 35 pounds independently.

Responsibilities

• Provide direct and indirect nursing care to PACE participants across care continuum.
• Perform nursing assessments and document per PACE standards.
• Coordinate care and participate in IDT meetings.
• Administer prescribed treatments, including IV therapy and wound care.
• Manage medications and coordinate with pharmacy; educate participants and families.
• Respond to emergencies and coordinate care across settings.
• Conduct home visits and participate in after-hours on-call rotation.
