Job summary

Huron Valley PACE in Monroe, MI seeks a Community Care RN to provide direct and indirect nursing care for PACE participants across the continuum of care.

The role requires an accredited nursing degree, MI RN license, CPR certification, and ability to perform complex assessments, coordinate care, and participate in IDT meetings. On-site with potential after-hours on-call responsibilities, IV therapy, wound care, and patient education are required.

Qualifications

• Graduate of an accredited School of Nursing.
• Current Registered Nurse (RN) license in the State of Michigan.
• CPR certification required (or ability to obtain upon hire).
• Valid Michigan driver’s license with reliable transportation.
• Ability to lift and move up to 35 pounds independently.

Responsibilities

• Conduct nursing assessments and document according to PACE standards.
• Triage participant concerns by phone and determine interventions.
• Develop and update individualized care plans with IDT input.
• Administer prescribed treatments and procedures, including IV therapy and wound care.
• Coordinate participant care across settings and track hospital admissions and discharges.
• Participate in home visits and after-hours on-call rotation.
• Follow workflows, delegate care, and support quality improvement.
