Job summary

NextStep Care is hiring for a community‑based care manager in Macon, GA. You will coordinate between members, families, and home/community providers to ensure high-quality, cost-effective care that improves health outcomes.

You will educate families about available resources, develop collaborative relationships with primary care providers, and monitor member wellbeing through calls and visits. Strong documentation, teamwork, and on‑call rotation participation are required.

Qualifications

• Bachelor’s degree in social work or related field with 2 years of experience.
• Experience in social work, HCBS, healthcare or geriatrics preferred.
• Ability to read and write clearly and communicate effectively.

Responsibilities

• Coordinate with members and providers to ensure quality, cost‑effective care.
• Educate members and families about resources and services.
• Develop collaborative relationships with primary care providers, HCBS and caregivers.
• Maintain regular contact with members via calls and home visits.
• Document interventions accurately and in line with guidelines.
• Support continuity of care with internal teams and quality staff.
• Participate in after-hours on-call rotations as assigned.
