How you will make an impact:

Red Cedar Canyon is looking for a Community Care Manager/LPN. Working within Vista Prairie Communities’ core values of Caring, Respect, Collaboration, Innovation, and Stewardship, the Community Care Manager/LPN is responsible for building on and helping residents find purpose, connecting them to their strengths, and enhancing their quality of life. This role involves leading, developing, and training direct care staff, creating and managing schedules, and monitoring service delivery to residents. You will deliver and manage care through the Aspire Lifestyle Model, engaging residents using the Aspire 5 pillars of wellness: Social, Physical, Intellectual, Emotional, and Spiritual.

Resident care responsibilities include responding promptly to emergency calls, assisting with transportation, activities, meals, and social programs. Staff responsibilities include scheduling, onboarding, mentoring, and documenting changes in resident conditions or staffing.

As an LPN, your duties will include:
  • Assisting with orientation, training, and mentoring new staff.
  • Supporting service startup, transitions, and discharges.
  • Performing delegated tasks and other duties as assigned.
  • Planning, assigning, and evaluating staff work, ensuring duties are performed promptly.
  • Monitoring resident needs and conditions, conducting assessments, and reporting findings.
  • Maintaining proper charting and documentation.
  • Reporting changes or concerns regarding residents.
  • Reviewing care plans under supervision.

Schedule:

Full-time, Monday-Friday days, on-call rotation.

What you will need:
  • Current Wisconsin LPN license in good standing.
  • Training or experience in health-related disciplines.
  • Must be 18 years or older.
  • Experience managing/leading a large team with complex scheduling, mentoring, and employee engagement.
  • Excellent communication skills, both written and verbal.
  • Proficiency with technology.
