Job Overview

The LPN Medication Care Manager is responsible for providing quality care by administering medication and treatments safely. Responsibilities include medication administration, documentation, conducting SHUs, and providing resident care in accordance with laws, standards, and Sunrise policies to ensure high-quality services.

Responsibilities & Qualifications

Essential Duties

• Medication Administration: Administer medications, review and document in electronic health records, ensure timely medication passes, maintain confidentiality, and report concerns.
• Resident Care: Address resident and family concerns, participate in care planning, and conduct service updates.
• Risk Management and Safety: Ensure compliance with safety regulations, practice infection control, and report hazards or exposures.
• Training & Team Contribution: Participate in team activities, meetings, training, and may supervise others.
• Core Competencies: Make decisions in residents' best interest, remain calm, handle priorities, communicate effectively, and demonstrate good judgment.

Experience & Qualifications

• Licensed nurse credential (LPN, LVN, or equivalent)
• Maintain required certifications (CPR, First Aid)
• At least 18 years old
• Experience with seniors preferred
