As we expand our clinical operations to support a growing patient and client base, we are seeking full-time registered nurses (RNs), licensed vocational nurses (LVNs), and licensed practical nurses (LPNs) to join our remote care team. This role plays a key part in our remote physiological monitoring (RPM) program by providing ongoing clinical oversight, developing patient care plans, and offering support and education to help patients improve their health outcomes.

Responsibilities:

• Review patient vitals and care plans within our proprietary technology platform
• Develop and manage personalized care plans addressing clinical, social, and behavioral needs
• Respond to clinical alerts to support improved health outcomes
  
• Provide patient education and guidance via video, phone, and asynchronous communication
  
• Coordinate care with physicians and act as a liaison between patients and providers
  
• Support patients in building healthier habits over time

Required Qualifications:

• Active and valid registered nurse (RN), licensed vocational nurse (LVN), or licensed practical nurse (LPN) license in the state of Michigan
• Comfortable using technology in a remote work environment
  
• Excellent communication and customer service skills

Preferred Qualifications:

• Experience in case management
• Bilingual or multilingual proficiency
• Compact state license
