Job summary

AbsoluteCare Medical Center & Pharmacy is seeking an experienced Complex Care Manager to lead a community pod and provide direct case management for highly complex members. You will coordinate care plans with CHWs, develop PCCPs, and serve as the clinical escalation path within a value-based care framework.

The role requires an active RN license, 3+ years of experience with medically complex populations, and willingness to travel up to 80% to member homes and facilities.

Qualifications

• Must be a licensed RN in the practicing state.
• Willing to travel up to 80% to member homes and facilities.
• CCM credential preferred; LMSW considered; CCM within 24 months of hire.
• 3+ years serving medically complex populations with behavioral health and social barriers.
• Experience leading or supervising allied health team members.
• Strong understanding of value-based care and quality programs (NCQA, PCMH, HEDIS).
• Excellent computer skills and written/oral communication abilities.

Responsibilities

• Lead the community pod as clinical and operational lead for care management.
• Coordinate care plans with CHWs and develop PCCPs.
• Oversee and coordinate care for an assigned panel within a region.
• Provide direct clinical care for a small panel of the most complex members.
• Lead daily morning huddles to review activities and KPIs.
• Oversee quarterly care plan oversight visits and escalation for CHWs.
• Participate in population health activities and panel reviews.
• Ensure documentation and communications meet agency and regulatory guidelines.
