Job summary

AbsoluteCare Medical Center & Pharmacy seeks a Certified Case Manager to join our interdisciplinary care team. You will assess members with complex medical, behavioral, and social determinants of health needs, develop person-centered care plans, and coordinate services across providers, plans, and community resources to support members with complex medical, behavioral, and social needs.

The CCM may visit telephonically, via telehealth, in the home or in outpatient settings, document interactions

Qualifications

Licensed clinician in the state where practicing.
3+ years serving the needs of complex populations.
Willingness to travel to meet members where they are.

Responsibilities

Attend member visits and provide follow-up support for care coordination.
Complete comprehensive assessment and PCCP for each member on the caseload.
Manage PCCPs and member contact in line with agency requirements and accreditation standards.
Develop, implement and maintain PCCPs using SMART goals.
Maintain up-to-date PCCPs in the electronic health record with objective measures to track progress.
Provide education with teach back regarding conditions, symptoms, and treatment options.
Provide evidence-based clinical interventions centered on PCCP goals.
