Job summary

AbsoluteCARE Medical Center & Pharmacy is seeking a qualified Care Coordination Manager to join the interdisciplinary care team. You will perform comprehensive assessments, develop person-centered care plans, and coordinate services across internal providers and health plans to achieve value-based outcomes.

The role requires 3+ years working with medically complex populations, strong communication, and proficiency with EMR systems.

Qualifications

Licensed clinician by the state and CCM credentials preferred; CCM within 24 months possible.
Minimum 3+ years serving complex populations in office or community settings.
Willingness to travel to meet members where they are (specialist offices, outpatient centers, etc.).
Proficient with Microsoft Office Suite and electronic medical record documentation.
Excellent written and oral communication and ability to meet accreditation standards.
Ability to work independently with sound clinical judgment.
Hold active driver's license and proof of insurance.

Responsibilities

Attend member visits and provide follow-up care coordination.
Develop and maintain PCCPs using SMART goals for each member.
Keep PCCPs updated in EMR with objective progress measures.
Provide education with teach-back on medical, behavioral, and functional health conditions.
Coordinate services with ICT, providers, and health plan programs.
Meet KPIs and manage caseload based on utilization data.
Provide crisis interventions as appropriate.
Assist members in accessing services and resources.
Document interactions and maintain schedule within 1 business day.
Actively participate in required meetings and referrals follow-up.
