Lutheran SeniorLife in Beaver County, PA is seeking a Community Care Coordinator (CNA or LPN) for a full-time, daylight shift. The role focuses on supporting participants in their homes and community through the LIFE program, coordinating services and promoting independence.

The ideal candidate has a GED or high school diploma, LPN license or equivalent, geriatric experience, plus strong computer skills, excellent communication, and a valid driver's license.

Qualifications

• High school diploma or GED required.
• Must have one (1) year of geriatric related experience.
• Working knowledge of computers, Microsoft Word and Excel required.
• Excellent interpersonal skills, team-oriented, knowledge of medical terminology, computer literate.
• Must have and present current valid driver's license.
• CPR and Certified First Aid required.

• Provides routine direct and indirect participant activities according to policies.
• Maintains participant rights, privacy, and confidentiality.
• Promotes participant acceptance of care plans and responds promptly to participant needs.
