Job summary

Lifeline Connections is seeking a Mental Health Case Manager to provide community-based behavioral health, care coordination, and re-entry support. You will work under supervision to develop Re-Entry Care Plans and coordinate access to a range of services to support recovery and community integration.

Required qualifications include a Bachelor’s in Behavioral Health and Washington credential or ability to obtain it, with at least one year of related experience.

Qualifications

• Bachelor's degree in Behavioral Health, Psychology, Social Work, Human Services, Counseling, or closely related field.
• Washington State credential required or ability to obtain and maintain credentialing.
• Minimum of one year of paid or volunteer experience in mental health/substance use.
• Working knowledge of trauma-informed care and participant-centered service delivery.
• Familiarity with RCW 71.05 and WAC 246-341.

Responsibilities

• Conduct comprehensive assessments including whole-person and Health-Related Social Needs (HRSN) assessments.
• Develop, implement, and monitor Re-Entry Care Plans aligned with goals and service needs.
• Coordinate access to behavioral health treatment, housing, transportation, benefits, and other services.
• Collaborate with multidisciplinary treatment teams and external partners to support outcomes.
• Maintain timely, accurate documentation in accordance with HIPAA and state regulations.
