Job summary

Maniilaq Association seeks a Behavioral Health Case Manager I/II/III to provide community-based support services for eligible recipients in Kotzebue, AK. You will help establish and coordinate treatment and social support services to meet recipient needs.

Responsibilities include outreach, referrals to therapy and services, coordination of care across providers, and maintaining timely documentation. Travel by small plane may be required; non-exempt status applies.

Qualifications

• High school diploma or equivalent required.
• Current Alaska driver’s license with insurable driving history.
• Excellent verbal and written communication skills.
• Ability to navigate multiple computer programs and document care plans.

Responsibilities

• Serve as central contact for connecting clients with mental health, SUD, medical, and social resources.
• Provide intensive outreach in community settings beyond the clinic.
• Refer clients for therapy, medical, or specialized services.
• Engage natural supports to improve client quality of life.
• Assist primary clinicians with assessments and treatment plan updates.
