Centerstone is a national nonprofit behavioral health system seeking a clinician to provide therapeutic interventions and develop personalized growth plans for clients with behavioral health diagnoses.

The role includes intake, assessment, care planning, documentation, and coordinating with internal and external resources, with emphasis on person-centered and family-centered care. Full-time position in Illinois with competitive benefits.

Formación

• Master’s degree in a human services field with licensure eligibility.
• Minimum 1 year of relevant experience in behavioral health.
• Ability to document and assess using standardized tools.

Responsabilidades

• Engages with clients and families and documents contact within 2 business days.
• Performs intake for new clients and ensures forms are understood and signed.
• Develops individualized care plans centered on person, family, and service goals.
• Assists clients in linking with internal and external resources as outlined in service plans.
• Maintains documentation of services and submits for review as required.
• Monitors client progress and participates in service plan reviews.
• Provides community support, care coordination, and crisis intervention.
