Job summary

Chartiers Center, Inc. seeks a Blended Service Coordinator to provide support and service coordination to adults with serious mental illness and co-occurring conditions in homes and community settings.

This role emphasizes compassionate, person-centered care and enables clients to achieve greater stability, independence, and recovery through integrated services and community connections.

Qualifications

• Associate’s degree in human services and 3+ years of mental health experience.
• Bachelor's degree in human services and 2 years of mental health experience OR Master’s degree in human services with one year of experience.
• Valid Driver’s License Required
• Ability to travel throughout the community

Responsibilities

• Meet with individuals in homes and community settings.
• Conduct assessments and identify strengths and needs and goals.
• Develop, implement and monitor individualized service plans.
• Connect individuals with housing, benefits, healthcare, behavioral health treatment, employment and community resources.
• Coordinate services among healthcare providers, therapist, family members and community agencies.
• Maintain regular communication with individuals to monitor progress and identify emerging needs.
• Advocate on behalf of individuals across multiple systems.
• Maintain timely and accurate documentation.
• Participate in an on-call rotation, typically no more than once per month.
