Job summary

TRILOGY BEHAVIORAL HEALTHCARE seeks a Relief Recovery Counselor to assess clients across personal, medical, and social domains, planning treatment through home, community, and office visits. You will provide strengths-based, trauma-informed care and link clients to needed community services.

The role requires crisis/on-call coverage and collaboration within a multidisciplinary team to advance recovery for individuals with severe mental illness and co-occurring substance use disorders.

Qualifications

• Must have a formal degree or credential in a relevant human service field.
• Must have licensure or credential as listed (LPN, CRSS, CPA, etc.).
• Experience with mental health and/or substance use preferred.

Responsibilities

• Assess client health, medical, emotional, social, and environmental needs during home, community, and office visits.
• Coordinate development of individualized recovery plans with clients and support systems.
• Identify levels of care and refer to additional services as indicated.
• Assist clients in recognizing decompensation signs and coping skills.
• Support independent living skills like hygiene, housing, nutrition, and budgeting.
• Help clients apply for entitlements (SSI, SSDI, Medicare/Medicaid, LINK).
• Accompany clients to appointments and advocate for housing and transportation needs.
• Advocate for clients and empower self-advocacy within a team approach.
• Communicate effectively within a multidisciplinary care team.
• Perform other related duties as assigned.

Licensed Practical Nurse (LPN) under Nurse Practice Act

Certificate of Psychiatric Rehabilitation (DHS IAODAPCA)

CRSS certification (IAODAPCA)

Family Partnership Professional certificate (IAODAPCA)

Occupational Therapy Assistant license

High school diploma or GED with 5 years supervised clinical experience
