Job summary

Qualifications

• Bachelor's Degree in Human Services, Social Work, Psychology, Counseling, or related field.
• Valid driver's license and reliable transportation.
• Ability to work effectively in both office and community-based settings.
• Intermediate computer proficiency, including Microsoft Office and electronic documentation systems.

Responsibilities

• Conduct comprehensive assessments to identify participant strengths, needs, barriers, and goals.
• Develop, implement, and monitor individualized service plans.
• Provide ongoing case management to support participant progress and success.
• Coordinate services across internal programs, healthcare providers, community agencies, and support systems.
• Facilitate referrals to treatment, recovery, and supportive services.
• Advocate on behalf of participants to access needed services, benefits, and resources.
• Assist participants with Medicaid, housing resources, employment and transportation support.
• Maintain documentation and ensure HIPAA compliance.
• Collaborate with families, treatment teams, and external agencies.
