Job summary

AbsoluteCARE Medical Center & Pharmacy is seeking a Community Based Advanced Practice Provider—Nurse Practitioner or Physician Assistant—in Columbus to deliver urgent and primary care in members’ homes and communities. You will partner with the member’s PCP and interdisciplinary team to facilitate safe transitions from hospital discharge and manage chronic conditions within a community setting.

Qualifications

• 2+ years’ experience as Nurse Practitioner/Physician Assistant with unrestricted license
• DEA license
• Experience across urgent care, ER, inpatient, office, or community-based settings preferred
• Familiar with care transitions, reducing readmissions and chronic condition management
• Experience with high-risk and medically complex patients preferred
• Psychiatric and substance use disorder experience helpful
• Extensive knowledge of local population, geography and resources
• Creative and innovative problem-solving to aid patients in overcoming barriers
• Ability to meet deadlines and manage multiple priorities
• Proficient with Electronic Medical Records and able to analyze/report
• Excellent computer skills, including Microsoft Office
• Patient-centered focus

Responsibilities

• Provide community-based medical and care-coordination services for recent hospital discharges
• Work closely with the transitional care manager and the member’s primary care team, following through on discharge plans
• Conduct patient assessments
• Review, monitor, adjust and discontinue medications
• Comprehensive Medication Review for adherence
• Patient/family education
• Coordinate home care agencies and services
• Develop and implement plan of care
• Communicate patient’s medical, or mental conditions, substance abuse and social determinants of health needs
• Document appropriately in the Electronic Medical Record
• Gather critical information from member’s home environment and coordinate with the care team
• Maintain safety and privacy of all information
• Support safe discharge transition and treatment in the community
• Intervene with at-risk members to avoid hospitalizations
• Coordinate with care team, patient/family, and caregivers to resolve barriers to care
