Job summary

CEI (Crandell's Enterprises) seeks a Peer Support Specialist to assist clients in implementing individualized treatment plans within the community. The role emphasizes active patient involvement, linkage to resources, and coordination of follow-up services to support stability and independence.

The position requires lived experience with the target population, a current peer support certification, and a high school diploma, plus driving and basic Microsoft Office skills.

Qualifications

• Current Peer Support Certification.
• High school diploma.
• Valid driver's license with a good driving record (preferred).
• CPR, First Aid, and NCI training.
• Working knowledge of Microsoft Office.
• Spanish speaking is a plus.

Responsibilities

• Ensure implementation of the individualized treatment plan (with active patient involvement) through appropriate linkages, referrals, coordination, followup to needed support services, and direct care services.
• Provide linkage to community resources.
• Assist clients with selfdetermination and decisionmaking.
• Teach functional skills for managing medications, health care, resources, daily living, and more.
• Model recovery values and personal action to encourage wellness.
• Teach and promote selfadvocacy and empower clients to use their legal rights.
• Maintain and submit required documentation, including treatment plans, service notes, and service authorization requests.
