Job summary

TRILOGY BEHAVIORAL HEALTHCARE in Chicago, IL seeks a Peer Support Specialist to assess clients’ personal, medical, emotional, social, and environmental needs and plan treatment through home, community, and office visits.

The role emphasizes motivational support and trauma-informed, client-centered services to individuals with serious mental illness and co-occurring substance use disorders using a harm-reduction approach.

Qualifications

• Must have one of the following: High School Diploma or GED with two years of supervised clinical experience in a mental health setting; Associate’s or Bachelor’s degree in any field.

Responsibilities

• Assess client’s personal, medical, emotional, social, and environmental situations through home, community, and office visits.
• Coordinate the establishment of an individual recovery plan with the client, support system, and other care providers.
• Provide client-centered, strengths-based, and trauma-informed services to clients with severe mental illnesses and co-occurring substance use/addiction disorders.
• Complete daily progress notes and documentation in the EMR per policy.
• Provide crisis/on-call coverage as directed.
