Oaks Integrated Care Inc. in Ventnor, NJ is seeking a dedicated Case Manager to join our independent housing program. This full-time role involves engaging and assessing consumers, developing personalized service plans, and coordinating across housing, education, employment, and health services.

You will provide life skills instruction, maintain electronic records, and submit routine reports while supporting transition goals and crisis interventions as needed.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Counseling, or related field with clinical training.
• Experience with youth living in foster care preferred.

Responsibilities

• Engage and assess consumers’ needs.
• Develop individual service plans to meet needs identified in housing, education, employment, health, and permanency.
• Provide life skills instruction through multiple methods of instruction.
• Assist consumers in accomplishing transition plan goals.
• Provide crisis intervention when needed.
• Provide behavioral interventions when needed.
• Maintain electronic health care records.
• Submit monthly, quarterly and semi-annual reports.
