Job summary

REM Community Services, a part of Sevita, seeks a Licensed Practical Nurse to provide essential nursing care and support within our community-based program. You’ll monitor health conditions, administer medications, and help implement ISP-driven care plans in a rewarding setting.

The role requires an LPN license, at least 1 year of nursing experience with the target population, and the ability to train staff on care procedures while collaborating with medical teams.

Qualifications

• Ability to effectively communicate with staff, individuals served, and families.
• Experience working with the population served in a nursing capacity is preferred.
• CPR/FA, HIV/AIDS, BBP, ACT training may be required by state licensing regulations.

Responsibilities

• Monitor the health conditions of individuals served and ensure quality services are provided.
• Administer medications and provide documentation as well as oversee staff administering medications.
• Provide prescribed medical treatment by physician's orders and personal care services.
• Conduct assessments and respond to changes of condition within ISP plans.
• Provide training to staff on infection control and medication administration.
• Serve as liaison with other medical and professional service providers.
• Provide training for individuals served on self-medication and related topics.
