Job summary

Keystone Health is seeking a Community Based LPN to provide direct patient care in home and community settings. The role supports telehealth visits and helps patients overcome medical and social barriers to access care.

You will work with an interdisciplinary team to coordinate short-term care, connect patients with resources, and deliver education and social support that promotes health management. Community outreach to underserved populations is a focus.

Qualifications

  • Graduate of an accredited Practical Nursing Program.
  • Licensed Practical Nurse with at least two years of ambulatory care or physician office experience.
  • Experience in community-based roles is a plus.
  • Current LPN license, CPR certification, and valid Driver's License.

Responsibilities

  • Deliver direct patient care in home and community settings.
  • Support Keystone Health providers during telehealth visits.
  • Facilitate access to care for patients facing barriers to medical and social services.

Community Based LPN
Full Time — Monday through Friday (8:00 am - 5:00 pm; occasional evenings/weekends for community events)

The Community Based LPN delivers direct patient care in the home and community, supporting Keystone Health providers during telehealth visits by performing prescribed treatments and facilitating access to care for patients facing medical, social, or transportation barriers.

Working as part of an interdisciplinary team—including medical providers, RN Care Managers, and Case Managers—the LPN provides short-term care coordination, connects patients with community resources, and addresses social drivers of health. The role also includes offering culturally appropriate education, social support, and informal counseling to help patients improve their health and self-management.

The LPN participates in community outreach and engagement activities to increase awareness of available services, with a focus on elderly and vulnerable populations.

Education & Experience
  • Graduate of an accredited Practical Nursing Program.
  • Licensed Practical Nurse with at least two years of recent experience in an ambulatory care or physician office setting.
  • Experience in community-based roles (health educator, community health worker, etc.) is a plus.
Licenses & Certifications
  • Current LPN license
  • Valid CPR certification
  • Valid Driver's License
