Qualifications

Responsibilities

• Perform functions within the scope of an LPN; maintain competency.
• Review and prepare Electronic Health Record (EHR) information prior to patient’s appointment.
• Prepare patient information for appointments by obtaining vital signs, reconciling medications, processing medication renewal requests, and documenting in the EHR.
• Assist providers with care and procedures.
• Carry out provider orders, including tests, medication administration and CLIA waived labs.
• Provide patient education and communicate test results and treatments.
• Collect samples (urine, stool, sputum, blood) for testing.
• Sterilize equipment and prep rooms between patients and at day end.
• Orient and mentor new hires and students as assigned.
• Attend department meetings and complete required training.
• Participate in organizational quality initiatives as requested.
• Understand KH programs and communicate information to patients.
• Assist with clerical tasks as necessary.
