Earn a living where your heart truly belongs.## **Community Based LPN****Full Time — Monday through Friday (8:00 am - 5:00 pm; occasional evenings/weekends for community events)**The **Community Based LPN** delivers direct patient care in the home and community, supporting Keystone Health providers during telehealth visits by performing prescribed treatments and facilitating access to care for patients facing medical, social, or transportation barriers.Working as part of an interdisciplinary team—including medical providers, RN Care Managers, and Case Managers—the LPN provides short‐term care coordination, connects patients with community resources, and addresses social drivers of health.
