Job summary

Trellis Supportive Care in Salisbury, NC is seeking a Hospice RN Case Manager – Homecare to deliver compassionate hospice care to patients in their homes and care facilities. You will develop individualized plans of care and coordinate with an interdisciplinary team to ensure comfort and quality of life.

The role requires nursing licensure, strong assessment and communication skills, and a compassionate approach to end-of-life care.

Qualifications

• RN license in good standing and hospice experience preferred.
• Strong assessment and critical thinking skills with comfort in end-of-life care.
• Excellent written and verbal communication with patients/families.
• Ability to coordinate care with an interdisciplinary team.
• Proven compassionate approach and cultural sensitivity.

Responsibilities

• Develop and implement individualized plans of care for hospice patients at home or facilities.
• Maintain comfort and quality of life while coordinating with the care team.
• Reassess and revise care plans as patient and family needs evolve.
• Supervise and collaborate with team members to ensure high-quality care.
• Communicate clearly with patients, families, and other providers.

Location: Serving patients and families in Rowan County and surrounding counties
Office Location: Trellis Supportive Care – Salisbury, NC

Schedule

• Full-Time (40 hours per week)
• Five 8-hour weekday shifts
• Occasional back-up on-call rotation
