Crossbridge Hospice is seeking an RN Case Manager to provide end-of-life care in patients' homes or community settings, directing hospice staff and ensuring compassionate, high‑quality nursing care aligned with patients’ goals.
You’ll collaborate with triage and clinical leadership, manage on-call caseloads, coordinate medications and supplies, and develop care plans using the 4Ms framework to support age-friendly care.

How you will contribute:
is responsible for providing end-of-life care and hospice services to patients in their home or community setting. This includes intermittent skilled nursing services and directs, supervises, and instructs hospice aide staff in the provision of personal patient care. This role requires compassion, strong clinical judgment, excellent communication skills, and the ability to support patients and families through highly sensitive and meaningful experiences.
