Diversus Health Inc. is seeking an Employment Specialist in Colorado Springs, CO. This full-time, in-person position focuses on helping clients obtain and maintain competitive community-based employment aligned with vocational goals.

The role emphasizes IPS services, collaborative planning with families, and building employer partnerships in the community. Responsibilities include needs assessment, job development, consumer education, and ongoing supports.

Responsabilidades

• Conduct functional needs assessment and determine IPS applicability.
• Engage clients with collaborative relationships to obtain/maintain employment.
• Assess long/short term employment goals and evaluate history.
• Educate family, friends and potential employers about clients' needs.
• Develop an IEP with client input and treatment team.
• Conduct job development and job search activities.
• Support clients in contacting employers within 30 days of intake.
• Provide follow-up supports to maintain employment.
• Negotiate workplace accommodations when needed.
• Attend meetings for continuity of care and employment sustainability.
• Attend community meetings with clients, job coaching, or development.
• Engage in treatment planning and evaluate progress.
• Maintain accurate documentation of services provided.
• Perform other job-related duties as assigned.
