Job summary

CareSource is seeking a Community Based Care Coordinator, Duals Integrated Care to manage and coordinate care for dual-eligible Medicare/Medicaid beneficiaries. Qualifications

• Nursing degree from an accredited program or a health care bachelor's degree.
• Equivalent years of relevant work experience may be accepted in lieu of education.
• Current, unrestricted licensure as RN, Social Worker, or Clinical Counselor.
• Case Management Certification is highly preferred.

Responsibilities

• Engage with members in community settings to build care coordination relationships.
• Liaise between providers, community resources, and dual-eligible beneficiaries.
• Conduct comprehensive assessments of physical, mental, and social needs.
• Develop and implement individualized care plans addressing medical and social needs.
• Lead interdisciplinary care team to create holistic plans.
• Assist members in accessing housing, transportation, and social services.
• Educate members about Medicare/Medicaid benefits and services.
• Monitor health status and adherence to care plans; adjust as needed.
• Follow up after hospitalizations to prevent readmissions.
• Coordinate with providers and state agencies to avoid service duplication.
• Document interactions, plans, and outcomes; analyze data for improvement.
• Advocate for dual-eligible beneficiaries and empower their participation.
