Qualifications

• Must have nursing or healthcare degree and relevant professional experience.
• Equivalent years of experience may be considered in lieu of formal education.
• Experience in care coordination or dual-eligible populations preferred.

Responsibilities

• Coordinate care for dual-eligible beneficiaries across settings.
• Engage members in community-based care coordination.
• Coordinate with providers and community resources for seamless transitions.
• Develop individualized care plans addressing medical, social, and behavioral needs.
• Lead interdisciplinary care team to address medical and non-medical needs.
• Educate members and families about benefits and services.
• Monitor care plans and adjust as needed to prevent readmissions.
• Maintain accurate records of interactions and outcomes.
