Job summary

United Cerebral Palsy Associations Inc. seeks an Integration Assistant to deliver community-based instruction and personal care to individuals with disabilities under supervision.

The role emphasizes safe transfer, health monitoring, and supportive ISP execution in both facility and community settings. You will record progress, coordinate with program staff, and assist with behavior modification as outlined in ISP.

Qualifications

• High school diploma or GED; experience in day or residential programs for persons with disabilities preferred.
• Ability to read, write and understand English; strong oral and written communication skills.
• Ability to perform basic math and use a computer.
• Must have a valid CDL and clean driving record.
• Ability to follow verbal and written instructions from supervisor.

Responsibilities

• Provide community-based group (1:3) daily instruction per Individual Service Plan (ISP).
• Record data daily on consumers' progress.
• Maintain communication with Program Manager, Instructor and/or Program Assistant.
• Ensure health and safety needs of consumers in facility and in the community.
• Assist with behavioral modification as specified in ISP.
• Provide input to supervisors on individualized program objectives.
• Provide personal care including toileting as needed.
• Promote teamwork and participate in staff meetings and training.
• Transport consumers daily.
