Job summary

United Cerebral Palsy Associations Inc is seeking an Integration Assistant to provide community-based instruction and personal care to individuals with disabilities in Stockton, CA. Under supervision, you will implement ISP-based activities, monitor progress, and maintain safety in both home and community settings.

The role requires reading, writing, English proficiency, basic math, and a valid CDL with a clean driving record.

Qualifications

High school diploma or GED; preference for experience in day or residential programs serving persons with disabilities.
Ability to read, write and understand English; strong oral and written communication skills.
Must have basic mathematical concepts and working knowledge of personal computers; CDL and clean driving record are required.

Responsibilities

Provide community-based instruction (1:3) and personal care following Individual Service Plans.
Record daily data on consumers' progress.
Maintain communication with Program Manager, Instructor, or Program Assistant.
Ensure health and safety needs of consumers in facility and community.
Assist with behavioral modification as specified in ISP.
Provide input on development of individualized program objectives.
Provide personal care including toileting as needed.
Promote a team-oriented work environment and attend trainings.
