Qualifikationen

• Knowledge of housing resources, employment supports, or behavioral health resources.

Aufgaben

• Provide person-centered case management for housing and employment supports.
• Conduct assessments to identify needs, barriers, goals, and readiness.
• Develop individualized service plans with clients and stakeholders.
• Assist with housing searches, landlord communication, and lease-readiness.
• Support clients with employment exploration, applications, and job readiness.
• Use motivational interviewing and strengths-based engagement to support goals.
• Provide transportation to appointments and events as needed.
• Spend 75-80% of time in the community meeting clients where they are.
